Social Services

Working as part of our health care team, hospital Social Workers are available to help arrange for care and services upon discharge from the hospital. Each care unit in the hospital has a licensed Social Worker on-staff to link patients and families to services that can help enhance a patient's function at home and in the community. This may include home-delivered meals, private home care or medical transportation and hospital van services to medical appointments.
