Our Practice
The Center for Geriatric Health and Disease Management coordinates health and social services for the elderly and their families focusing on comprehensive inpatient and outpatient care and education. It provides medical care for the older adult, assessment of memory disorders, a memory disorders program and education and support for the individual and their caregiver.
Our team provides the following services:
Primary Care
We provide long-term primary care for patients 65 years of age and older. We serve as an overall health care manager and, if necessary, working directly with specialists to determine the best treatment of care.
Outpatient Consultative Services
Our team assists primary care physicians with patients who have:
- Memory Impairment and/or
- Other Complex Geriatric Syndromes.
Cognitive Evaluations
We provide comprehensive evaluation and treatment for patients with concerns about memory impairment.
Inpatient Coverage of Our Patients
When our patients are admitted to the hospital, our team directs the care.
Inpatient Consultative Services
Our team provides assistance with the management of complex frail elders, especially those with cognitive impairment and/or delirium.
Housecalls
Primary care is provided for homebound seniors who reside in our service area (mainly Livingston, West Orange, South Orange and Short Hills).
Clinical Trials
The Center is actively involved in clinical trials in a number of areas including:
- Dementia
- Alzheimer's disease
- Other aging-related issues
