How Do I Sign Up for a MyChart Account?

There are several ways to register for a MyChart account.

• Click here to register now.
• Clinic staff can help sign you up directly while you're at the front desk or in the exam room.
• You will receive a MyChart activation code on your After Visit Summary you can use to sign up.
• You will receive a text or email with an activation code you can use when you come in for a visit.

Can I create a MyChart account for my child?

Parents or guardians of children under the age of 13 may act as a proxy for their child’s MyChart account — they can set up an account in the child's name. When the patient turns 13, the parent/guardian will no longer have access to the patient’s MyChart unless the patient grants them continued proxy access.
