Trauma Response Assessment and Coordination - Hospital Violence Intervention Program (TRAC-HVIP)
What Is TRAC?
The Trauma Response Assessment & Coordination (TRAC) Team is a hospital-based program at Trinitas Regional Medical Center (TRMC) offering comprehensive, integrated care coordination services with special emphasis on underserved populations and victims of violence.
Services include:
- Employment assistance
- Education opportunities
- Legal referrals
- Mental health services
- Community engagement services
- NJ Victim of Crimes Compensation Office (VCCO) applications
- Assistance with emergency subsidies
- Individual case management
- Advocacy
- Support with Food insecurity
- Support with Transportation
Who We Serve
- Union County residents,
- Age 18 and older,
- Individuals who have been admitted to TRMC for treatment of a violent injury resulting from interpersonal violence or community crime, and requires emergency medical care.
- Individuals who have been affected or have been a witness to interpersonal violence in the community
- Our team members are fluent in English and Spanish. Interpretation services are available if needed.
Purpose
We strive to restore adaptive capacity, facilitate healing, and decrease trauma for those who may have had barriers to treatment in the past due to issues with transportation, finances, and limited support, among other needs. All services are confidential and provided free of charge.
