What Is Transitional Care?
Transitional care is the continued monitoring and treatment of patients after they leave the hospital and transition back to their everyday lives. Even those recovering from short hospital visits should have the knowledge and resources they need to make their recovery as complete as possible.

Our goal with transitional care is to ensure that our patients have the tools and awareness they need to maintain their health. Our physicians, nurses, and counselors can help these patients identify medical warnings signs, manage a prescription regimen and schedule follow-up appointments over the course of their recovery. All patients will receive detailed contact information as well in the event they need to reach our medical professionals from home. We want to make sure our patients are cared for as they work to make a complete recovery.
Adult Transitional Care
Community Medical Center’s Transitional Care Unit in Toms River, New Jersey provides a temporary landing space for patients who no longer need all the resources of an acute care hospital, but still need nursing and rehabilitative care to recover.

Robert Wood Johnson University Hospital Rahway is home to Kindred Hospital, a long-term acute care hospital designed to focus on critically ill patients with serious and complex medical conditions that require intense specialized treatment for an extended period of time. It provides highly advanced and focused care for patients who have not responded to short term treatment in traditional general hospitals. The average length of stay is 25 days compared to a 3-to-5-day average in a general hospital. Patients appropriate for Kindred Hospital include those who are ventilator dependent, suffering from multi-organ failure or who have deep wound ulcerations.

Pediatric Transitional Care and Long Term Care

Adult Long Term Care and Rehabilitation Services
Sometimes patients whose acute conditions have been addressed by a hospital may require long-term care to address their continuing medical conditions.
Trinitas Regional Medical Center offers long term care in partnership with Brother Bonaventure Extended Care & Rehabilitation Center, a not-for-profit skilled nursing center in Elizabeth, New Jersey. We provide continuous care and support for individuals who require long-term care or sub-acute care before returning home.
With 24-hour licensed nursing care, we offer access to full hospital support services including the Wound Care Center, Dialysis Center, ophthalmology, podiatry, dental and psychiatric services, physical, occupational and speech therapies, Social Work services, therapeutic recreation and pet therapy; Pastoral Care programs and religious services; laundry services; and professional hairdresser, barber and specialized dietary services.
