All commercial drivers must undergo a comprehensive physical examination by a certified medical professional listed on the National Registry of Certified Medical Examiners (NRCME). What Is a DOT Physical?

A Department of Transportation (DOT) physical is a mandatory health examination required by the Federal Motor Carrier Safety Administration to ensure that commercial motor vehicle drivers are physically, mentally, and emotionally fit to operate a commercial vehicle safely. This comprehensive exam helps prevent accidents and injuries by verifying that drivers meet specific health and safety standards. Obtaining or renewing a Commercial Driver’s License (CDL) necessitates passing this exam, conducted by a certified medical examiner (CME), to confirm that drivers can perform their demanding jobs without posing a risk to themselves or others on the road.

How to Prepare for a DOT Physical

For your DOT physical or CDL medical exam:

  • Bring a complete list of your medications, including dosages, and a detailed medical history covering any past surgeries, medical conditions, and current treatments.
  • Bring corrective lenses or hearing aids if you use them.
  • Stay well hydrated and avoid caffeine and tobacco before the exam to ensure accurate readings.
  • Bring your current valid driver’s license and any necessary corrective eyewear.
  • Bring a completed Examination Report Form MCSA-5876, if possible.

What to Expect

  • Vision test. Ensure you have at least 20/40 vision in each eye with or without corrective lenses and can distinguish the colors of traffic signals.
  • Hearing test. Verify your ability to hear a forced whisper from at least 5 feet away, with or without a hearing aid.
  • Blood pressure and pulse. Your blood pressure and pulse will be checked to ensure they are within a normal range.
  • Urinalysis. Conducted to check for underlying medical conditions such as diabetes.
  • Physical examination. The examiner will check your eyes, ears, mouth, throat, heart, lungs, abdomen, extremities, and nervous system for abnormalities. This includes a neurological exam and a hernia check.
  • Mental and emotional health. Evaluating your mental and emotional fitness to handle commercial vehicle demands.

After the Physical

If you pass the exam, you will receive a Medical Examiner's Certificate (MEC), Form MCSA-5876, valid for up to 24 months. A certificate with a shorter validity period may be issued if certain medical conditions are identified. The Medical Examiner will keep a copy of your certificate on file for at least 3 years. All interstate CMV drivers must undergo a physical examination at least every 2 years, though the examiner may require more frequent exams if needed (e.g., annually or every 6 months).

As of May 21, 2014, all commercial drivers whose medical certificates expire must be examined by a Certified Medical Examiner listed on the National Registry of Certified Medical Examiners. The Medical Examiner will determine if you meet the physical qualification standards to drive a commercial motor vehicle.
