  • Group Diabetes Self Management Education
    Please note, in person diabetes education and medical nutrition therapy are offered individually. This class is for adults only. A physician’s prescription is required. We participate with insurance plans. Medicare reimbursement is also accepted. Pre‐registration is required by calling 973‐322‐7007.

Medical Nutrition Therapy

Nutrition counseling on an individual basis provided by a registered dietitian and/or certified diabetes educator. During an initial appointment the nutritionist will assess your medical history, dietary intake, exercise habits, blood glucose monitoring and your diabetes care plan. The nutritionist will then educate you on diabetes and develop an individual meal plan to meet your specific goals.

Gestational Diabetes Counseling

Gestational Diabetes counseling is done on an individual and/or group basis. Counseling is provided by a registered dietitian and/or certified diabetes educator. During an initial appointment the nutritionist will do an assessment of your medical history, weight gain, dietary intake, exercise habits and blood glucose results if available. Education on using a glucometer will be provided. The nutritionist will develop an individualized meal plan and educate you on achieving good blood sugars and appropriate weight gain for a healthy pregnancy.

Blood Glucose Monitoring

If you have diabetes, monitoring your blood sugars is an essential part of diabetes self management. Blood glucose monitoring helps you to make decisions on your food choices and helps you to see how your lifestyle affects your blood sugars. Blood glucose monitoring gives your doctor information on how well your diabetes is being managed by your diet, lifestyle and medication.

A certified diabetes educator will educate you on what is the best meter for you. We will educate you on the use of the meter and what the numbers mean. We will educate you on low blood sugars and high blood sugars and how to manage both conditions.

Prediabetes

Over 96 million American adults have prediabetes – that’s 1 in 3 adults! Of those, more than 8 in 10 of them don’t even know they have it and aren’t aware of the long-term risks to their health. Without taking action, many people with prediabetes could develop type 2 diabetes within 5 years. The good news is that prediabetes can be reversed.

The Center for Diabetes Wellness and Prevention offers Prevent T2 -- a year-long program recognized by the Center for Disease Control that has been shown to delay or prevent Type 2 diabetes.
