We are pleased to provide health services to lesbian, gay, bisexual, transgender, intersex and asexual (LGBTQIA+) adults. The PROUD Center is a safe and supportive environment where patients age 18 and older can get the medical care they need in an environment that promotes dignity and respect. Please fill out the form below and we will contact you to schedule an appointment.

In order to provide the most comprehensive medical care, please provide copies of any existing medical records to our office before your first visit. The staff cannot request medical records on behalf of a patient. Please contact your previous provider(s) to obtain copies of your medical records.
