Description:
Within 48 hours of booking confirmation, patients will be contacted by Robert Wood Johnson University Hospital Somerset’s Pre-Admission Testing department to schedule their Pre-Admission Testing appointment.
Patients will need to bring Robert Wood Johnson University Hospital Somerset’s Pre-Admission Testing form (21-368) from their physician for required testing as well as a form of identification and insurance cards.

Location:
Pre-Admission Testing is located on the ground floor, next to Patient Access.

Hours of Operation:
Monday – Friday: 8:00 AM – 4:30 PM, with the last scheduled appointment time of 3:00 PM.

Forms

OR Scheduling (21-010) OR Booking Form – This is the form that needs to be completed and faxed to the OR Booking Department to schedule surgical procedures. The OR Booking Department will confirm the booking by faxing the form back to the Surgeon’s office within 48 hours.

Informed Consent for Operations (21-744) – This is the most current consent form that needs to be signed by the physician and the patient and returned to Robert Wood Johnson University Hospital Somerset no less than 72 hours prior to the day of surgery.

Physician Order Form (83256) – This form can be used for Pre-op and/or Post-Op orders. If you would like to customize your orders, please contact Jennifer Clinedinst, Director of Health Information Management at 908-685-2200 ext. 2914 for approval.

Pre Admission Testing (21-368) – This is the form that needs to be used for all patients requiring pre-admission testing.

Progress Record (21-090) – This is the current version that is available in the Post Anesthesica Care Unit (PACU) at Robert Wood Johnson University Hospital Somerset.

History and Physical (H&P) (23071) – This H&P can be used for all patients coming through Perioperative Services.

Preoperative Medical Consultation (21-148) – Complete within one week prior to surgery.
