As one of the few centers able to successfully treat complex hernias that have failed previous repair, we are contacted by prospective patients throughout the country. To expedite the answer to the question - “Can you help me?” - we suggest you gather the following information before you call.

• Download and complete the patient history and physical form (PDF).
• Obtain a disc copy of an abdominal and pelvic CT performed within the last six months.
• Obtain copies of ALL operative reports.

When you call the Center, you will be asked for demographic and insurance information. If you are out of state or live more than two hours away from the center, located in Long Branch, N.J., you have the option of submitting the above required information to the Center for our surgeons’ review. If it is determined that you are a candidate for surgery, please contact our Concierge service representative, Virginia Costello at (732) 923-6742 for information on flight arrangements, area hotel accommodations, restaurants, etc.
