Care Coordinator

To serve as a link between patients, physicians, and David S. Zocchi Brain Tumor Center, the care coordinator ensures that the appropriate education, support and resources are provided. Which includes:

• Consultation:
  • Describe the comprehensive spectrum of services available at David S. Zocchi Brain Tumor Center for patients and families.
• Assessment:
  • Perform the detailed assessment to identify the individual needs for patients and families, and provide the education, information, and resource material.
• Follow-Up Care:
  • Conduct follow-up to reduce stress and provide ongoing support.
  • Assist with the coordination and scheduling of tests and services to ensure they are done in a timely and cost-effective manner.
• Communication:
  • Communicate with physicians, case managers, support services personnel, families and others to ensure that patients receive necessary services
