Projects for Assistance in Transition from Homelessness

• For individuals who have a history of mental health problems and are homeless or at serious risk of becoming homeless
• A co-occurring substance abuse problem may be present, but the mental health issue needs to be the primary problem
• Services include case management with the goal of linking individuals to as many services as possible in the community, such as mental health and substance abuse treatment services, counseling, medical and dental care, financial assistance, temporary and permanent housing, habilitation and rehabilitation programs (all subject to availability)
• Psychiatrist on-site to assist with establishing eligibility for the program and to discuss medication options

MASSH/PATH is 100% grant-funded through grants from the Division of Mental Health and Addiction Services and is part of the Behavioral Health Department at Jersey City Medical Center-RWJBarnabas Health
