Description: PRP, one of the largest providers of hospital-based recovery support services in the United States, uses evidence-based, patient-centered approaches to provide 24/7 hospital-based, ambulatory, and community recovery support services for patients with substance use disorder (SUD) in all RWJBarnabas Health (RWJBH) acute and ambulatory settings. The program staffs 108 full-time hospital- and community-based NAADAC-certified Recovery Specialists, 12 Patient Navigators, and 11 Case Managers.
Project/Program: Emergency Department Alternatives to Opioids Program (ED-ALT)

Description: Implemented at NBIMC, ED-ALT expands and utilizes evidence-based approaches to treat patients with painful conditions in the emergency department (ED) with opioid alternatives. The program utilizes a comprehensive education and training program for clinical staff and patients to help better treat and manage chronic pain.
Project/Program: Screening, Brief Intervention, and Referral to Treatment (SBIRT)

Description: In partnership with the RWJBH Medical Group, IFPR will implement a screening process for all patients 12 years or older for risky or unhealthy substance use and provide a brief in-office intervention, brief treatment, and/or refer patients to PRP for recovery services, when applicable.
Project/Program: Medication Assisted Treatment – Prescription Drug and Opioid Addiction (MAT-PDOA)

Description: Implemented at NBIMC and CMMC, MAT-PDOA expands access to medication for opioid use disorder (MOUD), enhances recovery support services, and provides patients with transitional medication in between discharge from the hospital and placement in a community care setting.

Funding: Reducing Deaths Due to SUD

Description: Utilizing a peer recovery approach, PRP-Maternal Health partners with hospital and community-based organizations to implement evidence-based interventions that strengthen perinatal and postnatal support services for patients with SUD, connect patients with peer recovery specialists who are cross-trained as doulas, address social determinants of health (SDOH), and reduce deaths among perinatal and postpartum patients.

Description: The Collaborative is a person-centered network of community-based organizations that brings countywide resources together to provide streamlined, comprehensive case management and recovery support services to Monmouth County residents with SUD and their families.

Funding: Tobacco Community Prevention and Cessation Services - Quit Center
Locations: Monmouth, Ocean, Essex, Middlesex, Union, and Mercer counties
Description: Tobacco Quit Centers in Monmouth, Ocean, Essex, Middlesex, Union, and Mercer counties provide patients with eight weeks of free tobacco recovery support, including free nicotine replacement therapies such as nicotine patches, gum, and lozenges with no insurance required and individual and group support meetings.

Description: IFPR’s Data and Analytics team develops reports based on data collected by and for the MCOFRT and disseminates information and findings including decedent demographics, SDOH, Adverse Childhood Experiences (ACEs), and access to support services. IFPR’s Data and Analytics team, in collaboration with the MCOFRT, provides critical recommendations to further enhance substance use services throughout Monmouth County.

Description: DART, one of 17 DMHAS-funded regional prevention coalitions, has served the Ocean County community since 2009, engaging residents in the process of reducing youth substance use, with a focus on reducing underage drinking, non-medical use of prescription drugs, illicit use of marijuana, and illegal use of tobacco and nicotine products. DART currently oversees 20 Youth Prevention Coalitions (YPC) with over 250 members in middle and high schools across Ocean County.
Project/Program: Southern Ocean County Interfaith Advocacy and Leadership (SOCIAL) Network

Description: An offshoot of DART, the SOCIAL Network is implemented in southern Ocean County and convenes representatives from faith-based organizations to equip faith-based leaders with the tools needed to build capacity, strengthen substance use prevention, and address SDOH within their communities.

Location: Southern Ocean County, specifically in Little Egg Harbor, Tuckerton and Eagleswood
Description: IFPR’s Southern Ocean County Interfaith Advocacy and Leadership (SOCIAL) Network will implement the DFC Support Program to reduce underage alcohol and e-cigarette use among youth 18 years of age and younger in Southern Ocean County, specifically in Little Egg Harbor, Tuckerton and Eagleswood. By building on the SOCIAL Network’s existing partnerships, the team will implement a comprehensive, evidence-based work plan, which includes disseminating informational tools such as signage, toolkits and stickers; enhancing knowledge and skill-building through trainings for alcohol retailers, youth and adults; increasing opportunities for youth leadership through youth-led initiatives and peer-to-peer support; and increasing access to community resources and activities.

Description: In Ocean County, adults and staff of youth-serving organizations, first responders, teachers, and other caregivers are trained in Youth Mental Health First Aid (YMHFA) to identify signs of youth mental health and substance use challenges and provide appropriate community referrals. High school students in grades 10-12 are trained in Teen Mental Health First Aid (tMHFA) to recognize mental health challenges in their peers, provide peer support, and build awareness alongside trained adults in schools.

Description: CTC is an evidence-based and data-driven process that addresses risk factors within each community to reduce and prevent behaviors including substance use, mental health, and violence and engages the community by employing coalition-building to drive decision-making and foster leadership.
Project/Program: Lesbian/Gay/Bisexual/Transgendered or Queer/Questioning(LGBTQ)+ Youth Prevention Program

Description: Through DART YPCs, the program provides substance use prevention services to LGBTQ+ youth in middle and high schools throughout Ocean County and the Southern Region of the state to develop an LGBTQ+ prevention infrastructure that creates positive health outcomes for LGBTQ+ youth.

Description: The Anti-Stigma Initiative aims to improve community responses to individuals with SUD and actively combats the harmful stigma associated with addiction. Through this program, IFPR’s prevention team works with clinical and community partners throughout Ocean County to better identify and address addiction stigma in our communities and implement substance use awareness and educational social media campaigns.
