Diabetes Self-Management Classes

Community Medical Center’s Center for Diabetes offers a 4-session diabetes education program to help those diagnosed with diabetes understand the disease, how to manage it, and tips on what to eat to maintain good blood sugar control.

Two upcoming class series have been announced:

Fridays, 10 am – 12 pm – starts July 19

Thursdays, 5:30 – 7:30 pm, starts September 5

Classes are held in Community Medical Center’s Center for Diabetes, conveniently located next to the hospital at 9 Hospital Drive, Suite B-15, Toms River, NJ 08755.

Advanced registration is required by calling 732-349-5757. To register, you must have a prescription from your doctor for ‘Diabetes education’ with a diagnosis code. Please have your health insurance information available when calling to register. The class will be billed to your insurance provider. Attendees may bring a guest with you at no additional charge.

Week I/Class I:

  • Pathophysiology of diabetes
  • Types of Diabetes
  • Adjusting to self-management- family/ social support
  • Identification of nutrients –carbohydrate, protein, fat; effects of nutrients on blood glucose
  • Importance of self-blood glucose monitoring; blood glucose goals
  • Carbohydrate counting
  • Building a meal plan- Breakfast

Week II/ Class II

  • Medications for the treatment of diabetes
  • Hypoglycemia- Low blood sugars
  • Hyperglycemia- High blood sugars
  • Carbohydrate counting, portion guidelines
  • Building a meal plan- Lunch

Week III/Class III

  • Sick Day guidelines, stress management
  • Prevention/treatment of chronic complications
  • Foot Care
  • Coping with long-term illness
  • Making heart-healthy food choices
  • Building a meal plan- Dinner

Week IV/Class IV

  • Tracking blood sugars / Hemoglobin A1c
  • Weight management guidelines
  • Dining out
  • Benefits of Exercise- Effects on Blood sugars
  • Overall Health Plan
  • Healthy snacking
