Center of Excellence for Latino Health
The Center of Excellence for Latino Health (CELH) at Clara Maass Medical Center was established with generous support from the HealthCare Foundation of New Jersey, Verizon and Partners for Health to serve as a hub of health and wellness information for Latino families. We focus on addressing the unmet needs of the local Latino community for specific disparities of medical conditions, such as diabetes, hypertension, hyperlipidemia, and obesity and cancer. Our Health Ministry is dedicated to assisting religious leaders, their families and congregations by connecting them to health care providers and services.
Our goal is to improve health care delivery to the local Latino community by:- Providing and facilitating primary, emergency and specialty, inpatient and outpatient medical care
- Focusing on transitions of care, chronic disease management and end-of-life care
- Looking at clinical and market research to understand patient needs and improve health outcomes
We plan to:
- Ensure that Latino patients seen at CMMC are aligned with a Primary Care Physician (including Pediatricians and OB/GYNs), to ensure consistent medical care
- Create a directory of health care sources including primary and specialty providers, outreach workers, health educators, social workers and other community organizations that provide needed services
- Assist with enrollment into quality insurance products through the NJ State Health Insurance Exchange program, as well as NJ Family Care (Medicaid)
- Develop a Latino youth mentorship program that will touch on a variety of health care issues and promote health care career opportunities
