At Clara Maass Medical Center, we’ve found a new way to provide more personalized care especially for our ventilated patients.
By having the family members complete a ‘Get to Know Me' form, our patient care team will be able to enhance communication, get to know each patient through basic information such as nicknames, important people, pets, fun facts, personal preferences and more.
We invite families of our ventilated patients to complete the form below and provide information with as much details as you’d like, and to please feel free to upload a picture or two.
