RWJBarnabas Health is seeking a Subacute Hospitalist to support our Skilled Nursing Facilities in Mercer County, New Jersey.
Full Time, Part Time, or Per Diem Positions are available!
This is a unique opportunity to practice with a flexible Monday-to-Friday schedule, achieve an outstanding work-life balance, and align with the largest integrated healthcare system in New Jersey.
The Subacute Hospitalist will manage post-acute and chronic diseases, diagnose and monitor medical conditions and illnesses, and implement patient care plans. They will also collaborate and coordinate care with other physicians, advanced practice providers, facility leaders, ancillary services, and support staff.
Ideal candidates will have a strong work ethic and a desire to participate in a team-oriented busy practice. They will also engage with sub-specialists, case managers, social workers, and all others involved in the patients' care, discharge planning, and patient/family communications.
Candidate Requirements:
- Board Eligible or Board Certified in Internal Medicine or Family Medicine
- Licensure or eligibility for licensure in the State of NJ
- Additional training and/or certification in Geriatrics preferred
- Flexibility to travel between Skilled Nursing Facilities
- Ability to participate in a shared group call model
