NCQA Patient-Centered Medical Home™ standards emphasize
enhanced care through patient-clinician partnership

NCQA Patient-Centered Medical Home™December, 2020, New Jersey — RWJBarnabas Health Medical Group (RWJBH) announces several of their practices have received National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition. The NCQA Patient-Centered Medical Home standards emphasize the use of systematic, patient-centered, coordinated care that supports access, communication and patient involvement. It was developed to assess whether clinician practices are functioning as medical homes and recognize them for these efforts.

The NCQA Patient-Centered Medical Home program reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others. To earn this recognition, which is valid for one year, RWJBH Medical Group providers demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home.

“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that these RWJBarnabas Health Medical Group practices have the tools, systems and resources to provide its patients with the right care, at the right time.”

In addition, several of RWJBarnabas Health Medical Group practices listed below are the only sites in New Jersey to have received NCQA’s Distinction in Behavioral Health Integration which recognizes primary care practices that put resources, protocols, tools and quality measures in place to support the broad needs of patients with behavioral health related conditions.
