Care, Support and Compassion When You Need Them the Most
Everyone deserves access to high quality health care and no one should be denied the medical services they need because they are unable to travel to the hospital. Through our partnership with the Visiting Nurse Association (VNA) Health Group, we will work to bring health care to you to improve the quality and efficiency of post-acute care and home care in the communities we serve.
If you or your loved one are in need of extra assistance while recovering from surgery, recovering from a serious illness or injury, or going through the final stages of life, we will work to ensure that you have everything you need to live as comfortably and pain free as possible.
Home Care Services
Our home care services are Medicare and Medicaid-certified. These services are available to patients who are unable to travel to the hospital and need extra assistance in the household. Our visiting nurses and physicians work closely with patients’ primary care physicians when coordinating care to ensure all of their needs are addressed.
Home care services may include:
- Visits from a registered nurse
- Physical, occupational and speech therapy
- Dietary assistance
- Wound care
- Pain management
- Pediatric care
- Social work
- Infusion therapy
- Recovery programs for joint replacement, heart failure and other conditions
We create a personalized home care plan based on the needs of the patient. Whether your loved one requires a full-time live-in assistant or just occasional visits from our home care team, we can connect you with the resources you need.
