Programs
The Center for Comprehensive Care provides comprehensive medical services and case management to children, adolescents and adults infected with HIV in Jersey City/Hudson County. Currently, funding is provided by grants from the New Jersey Department of Health, Division of HIV/AIDS Services, the New Jersey Department of Health Special Child, Adult and Early Intervention Services and the Ryan White Care Act (Part A, Part B and Part D). These services are provided within a multidisciplinary team approach, which ensures that medical, case management, and supportive services are received in the most comprehensive and cost effective manner. The goal of the treatment team is to promote the health and independence of the client by fostering treatment adherence and offering psychological support and intervention.
Our endocrinologists are dedicated to our patients, and the evaluation, management and research of a multitude of endocrine disorders including: thyroid abnormalities, bone and calcium metabolism, disorders of growth hormone, complications of menopause, hirsutism, polycystic ovary, hypogonadism, sexual dysfunction, infertility, lipid metabolism, endocrine hypertension, adrenal, pituitary and diabetic disorders.
Green Apple Rx, JCMC Food FARMacy
Jersey City Medical Center created Green Apple Rx, JCMC Food FARMacy to serve community members living in Hudson County who are food insecure and have a chronic disease such as diabetes, obesity or hypertension.The program, which includes nutrition counseling, is free, but registration and participation with our Greenville Primary Care Team is required. Mental Health and Social Services for the Homeless
Mental Health and Social Services for the Homeless/Projects for Assistance in Transition from Homelessness (MASSH/PATH) is committed to providing homeless individuals with accessible, high quality care in a friendly environment. Services include case management with the goal of linking individuals to as many services as possible in the community, such as mental health and substance abuse treatment services, counseling, medical and dental care, financial assistance, temporary and permanent housing, habilitation and rehabilitation programs (all subject to availability). Psychiatrist on-site to assist with establishing eligibility for the program and to discuss medication options.
